Probabilmente edificato dagli abitanti di Riomaggiore in seguito ad una visione mariana che ebbe luogo nel 790, divenne in seguito meta di pellegrinaggio. Le prime notizie certe sul Santuario di Nostra Signora di Montenero si hanno a partire dal 1335, ma il suo aspetto attuale risale al 1847. Il culto religioso trae origine, secondo la tradizione, da un\u2019icona bizantina che non si voleva far cadere nelle mani dei pirati e che, a furia di sotterrarla allo scopo di nasconderla, nel XV secolo and\u00f2 perduta. It is believed that the first sanctuary was erected here by the inhabitants of Riomaggiore after an appearance of the Blessed Virgin Mary in 790, and then became a place of pilgrimage. The first confirmed evidence regarding the Sanctuary goes back to 1335, yet the building as we see it today dates back to 1847. According to tradition, the veneration of the Virgin Mary has its origins in a Byzantine icon hosted here: the inhabitants of the area did not want to let the icon fall into the hands of the pirates and they kept hiding it, until in the 15th it got lost.
